Purpose

The Client Information Sheet (CIS) can be used as a standalone document, or it is usually nested within a KYC (Know Your Client) package to provide contact and identification details of the Asset Holder.

When using a CIS outside of a KYC, typically you may not need a Transaction Code, unless it is being used as part of an asset submission package, where the buyer has requested minimal details such as a CIS instead of a KYC.

The KYC document is provided below in Microsoft Word and Mac Pages (v4.2) format such that it can be edited. The document was created on a Mac, and typically the formatting does not translate nicely to Microsoft Word. A PDF file is also attached to show how the formatting was intended.
